§ 46-41. Physical examination form.

Along with the application form mentioned in sections 46-39 and 46-40, each applicant for membership in the fire department shall have filled out and filed at least one form relating to his health and physical condition, which form shall be as prescribed by the chief of the fire department and on file in the office of the personnel department of the city.
